Terms and conditions for FIT4BABY Prenatal Membership

MONTHLY MEMBERSHIP AGREEMENT

This contract made between the client and FIT4MOM North Metro Denver is a month to month agreement and except as provided in the initial 10-day cancellation policy below may be terminated by either party with the proper advance notice. There will be an early cancel fee of $79 processed if cancelled before the 3 month commitment. This membership allows you to attend unlimited Stroller Strides, Stroller Barre, Strides 360, Body Boost, Body Ignite and prenatal FIT4BABY classes.

AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM North Metro Denver (NMD Mamahood, LLC) to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have canceled my membership in writing through the FIT4MOM North Metro Denver franchise owner.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying by bank. This, however, does not void my contract with FIT4MOM North Metro Denver to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned, they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid, it will be cancelled.


CANCELLATION POLICY

To cancel this membership, the member must contact the FIT4MOM North Metro Denver franchise owner at carolmalone@fit4mom.com at least 30 days prior to their next payment due date to request a cancellation. Cancellation will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the North Metro Denver franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee. Instructors are not allowed to cancel memberships.

HOLD POLICY

Memberships may be held a maximum of 6 months per hold. To place a hold on this membership, the member must contact the FIT4MOM North Metro Denver franchise owner at carolmalone@fit4mom.com at least 10 business days prior to their next payment due date to request a hold be placed on their membership. Holds will be confirmed in writing to the member once they have been processed. Hold starts will coincide with the next date dues is scheduled to be drawn out on and can end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume on the return date. It is up to the client to keep track of their return date. There will be a $20 processing fee charged to your credit card on file for the hold period. This $20 will be credited to your account once your hold ends. Instructors are not authorized to place a hold on memberships.

INITIAL 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM North Metro Denver within the initial ten days. You are responsible for paying for the classes already attended.


FIT4MOM ON DEMAND

With your paid monthly membership you are given free access to FIT4MOM On Demand. You will have unlimited access to On Demand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your On Demand subscription will be automatically canceled. If you would like to keep your On Demand subscription you can re-enroll using the same login information and purchase your desire paid subscription.
